INSTRUCTIONS HOW TO COMPLETE PART I OF FORM 8889 FOR 2010

Every year, I receive a number of questions regarding how to complete Part I of Form 8889. The reason for this is that the instructions to Form 8889 are very complex and unclear. To assist, the following shows how to complete Form 8889 in 27 different situations for 2010. I have added four (4) situations this year. They are provided at the end as situations XXIV, XXV, XXVI and XXVII.

    Quick question on the Form 8889 for 2011. My wife had her own HSA (family coverage with her and my son) from Jan 2011 to April 2011. She was laid off and both her and my son joined my coverage from May 2011 to Dec 2011. So my coverage went from single to family starting in May. My question is, how many Form 8889’s do I need to fill out? Last year I filled out one for her and one for me, but this year, I am not sure how to do it because we both had family coverage, and I am not sure whether we have to split the $6150 between the two forms, etc. Any thoughts would be appreciated.

    Hi Larry Grudzien,
    This is Ziyan, I am new about the HSA and I have questions about how to fill up the form 8889.
    I am qualified the “Individual under age 55 with self-only High Deductible Health Plan (“HDHP”) coverage makes a contribution of $3,050 during 2011and 2012”
    My problem is I only contribted $1020 and spend all of them on the qualified medical expense. Could you please help me to file the fax?
