Introduction: Every year, I receive a number of questions regarding how to complete Part I of Form 8889.  The reason for this is that the instructions to Form 8889 are very complex and unclear.  To assist, the following shows how to complete Form 8889 in 23 different situations for 2009.

    a great resource, but I note one situation not covered: a person who lost their job and set up an HSA for the purpose of paying COBRA premiums, otherwise described as “Health care continuation coverage” in IRS Publication 969 (page 8), but apparently explained no where else.

    Thank you for your information, but my headache lies in completing Part II, particularly line 15. I am much like your first scenario. I funded my HSA roughly $2181 and I used that exact amount to pay for qualified medical expenses. I thought I set up this account so I would could claim a deduction on my after-tax dollars, however the 2009 instructions for line 15 (in part II) states I “cannot take a deduction on Schedule A (Form 1040) for any amount included on line 15”. If this is the case, then how do I claim my so-called deduction?

    Any help you can help is greatly appreciated!

    All the instruction guide lines are for 56yr and under. How do the instructions apply to 60yr or older.
